Q: How soon I can get an appointment?
A: We have same day appointments available for most of the times.
Q: What kind of Insurance plans do we accept?
A: We accept most of the commercial insurance plans as well as Medicare and Medicaid.
Call our office to verify if we are on your insurance plan.
Q: What should I do in case of emergency?
A: Please call 911 for life threatening emergencies. For urgent health problem call our office and try to talk to a physician. If it is after office hours, call our office phone number and call will be answered by our 24 hour answering service and they will page physician to get back to you.
Q: How much earlier should I come for an appointment?
A: If you are a new patient then arrive 10 to 15 minutes before your scheduled time. If You have an access to internet, go to Download Forms Section on our Website and download, print and fill out 'New Patient Registration Form' and 'Medical History Form' and bring them at the time of appointment. By doing this, You will save your time as well as our time at the time of registration. If you are an Established patient then you should arrive 5 to 10 minutes before your scheduled time.
Q: What should I bring at the time of appointment?
A: Current insurance card and a photo ID.
Bottles/list of current medications.
Any medical records available to you.
Co-payment if required as per your insurance plan. We accept cash, check and major
credit cards.
Q: What other services do we provide?
A: We have on site MRI, CT as well as Ultrasound. We also have Sleep study clinic on
our premises. We do some basic labs, EKG, ABI (Ankle Brachial Index)
measurement for diagnosing PAD (Peripheral Arterial Disease), Spirometry, Pulse
Oximetry etc. in our office.
Q: What if I need to get admitted to the hospital?
A: Dr. Thakkar has admitting privileges at Valley West Community Hospital in
Sandwich, Illinois as well as Rush Copley Medical Center in Aurora, Illinois.
Q: What should I expect after I am done seeing a physician?
A: If any blood tests done or any other diagnostic studies ordered at the time of visit, Please call our clinic in few days to find out about results of the tests if you don't hear from our office about them. Also maintain your follow up visit if given.
