How soon can I get an appointment?
Appointments are often available on the same day, depending on scheduling and availability.
What kind of insurance plans do you accept?
We work with many commercial insurance providers and also accept Medicare. Please contact our office to confirm if your insurance plan is included.
What should I do in case of an emergency?
For life-threatening conditions, dial 911. For other urgent medical concerns, call our office and request to speak with a physician. After regular hours, our phone line is connected to a 24-hour answering service, which will contact the doctor for you.
How much earlier should I come for an appointment?
New patients should arrive 10 to 15 minutes before the scheduled appointment time. If possible, download and complete the ‘New Patient Registration Form’ and ‘Medical History Form’ from our website and bring them with you. This helps speed up your check-in process. Returning patients should arrive 5 to 10 minutes early.
What should I bring to the time of the appointment?
Bring your current insurance card and a valid photo ID. Also, bring any medications you’re currently taking, along with relevant medical records. Co-payments are expected at the time of visit. We accept cash, checks, and major credit cards.
What other services do you provide?
Our facility includes MRI, CT, and ultrasound capabilities. We also offer sleep study services and conduct lab work, EKGs, ABI tests for PAD, spirometry, and pulse oximetry, all on-site.
What if I need to get admitted to the hospital?
Dr. Thakkar is affiliated with Valley West Community Hospital in Sandwich, Rush Copley Medical Center in Aurora, and Provena Mercy Medical Center in Aurora, Illinois.
What should I expect after I am done seeing a physician?
If tests or procedures were done during your visit, please call our office within a few days to check for results. Be sure to follow any return visit instructions provided.
